We must have your original, notarized release on hand at the ballpark before your child can participate.

If Opening Day is coming up soon, do not mail your form—we may not receive it in time. Instead, arrive 20 minutes before your child’s first game and hand it in at the ballpark. Check your email for your players team assignment and game time.
A notary public is an official who confirms your identity when you sign an important document. They check your ID, watch you sign, and then stamp or sign the form to make it official.
