Before you mail us forms

We must have your original, notarized release on hand at the ballpark before your child can participate.

If you’re sending it early enough, mail it to:

If Opening Day is coming up soon, do not mail your form—we may not receive it in time. Instead, arrive 20 minutes before your child’s first game and hand it in at the ballpark. Check your email for your players team assignment and game time.

What is a notary public?

A notary public is an official who confirms your identity when you sign an important document. They check your ID, watch you sign, and then stamp or sign the form to make it official.

You can find a notary at a bank, shipping store (like UPS), or city office. Some may charge a small fee.

To get your form notarized:

  1. Print the release form, but don’t sign it yet.
  2. Take your blank form and your photo ID to a notary.
  3. The notary will check your ID, watch you sign, and notarize the form.
  4. Later, turn in the original, notarized liability release to Buddy League.
